Who does the Community Care Fund serve?
The Community Care Fund primarily supports patients recently discharged from United Regional who face ongoing medical needs and limited financial resources. Many are referred to the Transition Clinic, where compassionate staff connect them with vital services tailored to their recovery.
What support does the Community Care Fund provide?
- Medications – Ensures access to prescriptions for patients with no ability to pay
- Transportation – Provides rides to medical appointments and treatments
- Diabetic Supply Kits – Includes insulin, syringes, and testing supplies for up to 60 days
- Durable Medical Equipment – Supplies items like oxygen, blood pressure cuffs, glucometers, and scales
- Food Boxes – Delivers nutritious food to patients struggling with food insecurity
- Access to Specialists – Connects patients with the specialized care they need
- Additional Support – Offers other critical services based on individual needs
Why is the Community Care Fund essential?
Due to federal restrictions, hospital funds cannot be used to cover critical supplies and services for patients under Medicare, Medicaid, or Tricare. That means many individuals are left without access to medications, diabetic supplies, or transportation after discharge.
